OverviewWork with a 22 year old Health Information Exchange organization, founded in 1996 and is still growing and expanding.The Santa Cruz health information exchange was the first in California (1996), one of twelve original health information exchanges in the United States, and one of the three that survive today. Initially formed as a local initiative by Dominican Hospital and the Physicians Medical Group, the Santa Cruz exchange grew to include 2,000 users from hospitals, practices, federally qualified health centers, and County agencies. In 2017, the IPA spun out the exchange, which became an independent 501(c)(3) public benefit company funded by initial capital and ongoing access fees and grants.Today, the Santa Cruz Health Information Organization (SCHIO) provides an array of services beyond "-- -Classic HIE' and serves approximately 400,000 patient lives in Santa Cruz County and from surrounding areas. We have recently expanded into social detriments of health (SDOH) and social referrals, such as housing, transportation, and food insecurities. SCHIO connects to the national networks, eHealth Exchange and Carequality, and connects to California's prescription drug reporting system, immunization registry, and disease reporting registry, as well as regional university laboratories and hospitals.Learn more about: SummaryThe Program Manager will be responsible for the planning and program implementation of the Together We Care (TWC) program. The Program Manager will ensure prioritization of SCHIO client requirements, documenting and refining the program vision, and partnering closely with the information technology assigned and it's several external client organizations to deliver client satisfaction and deliver workflows and to meet CalAIM needs. Includes the management of a process including related projects and resources as assigned. Based upon assignment, managers administer programs and projects supporting TWC assigned process with administrative functions. Will maintain consistent communication with board members providing for theacross-the-board first level management necessary to execute the process, programs, and projects of the organization. Will partner with all executive level leaders with the HIE and coordinate any essential meetings in respects to the TWC program. Will oversee day-to-day operational management of all activities and functions related to the TWC program. Which may include process/program(s) including developing, implementing and evaluating operating policies, procedures and standards; determiningservice levels and enhancements; developing and monitoring budgets; providing technical advice and/or oversight to staff or other departments related to area of assignment; evaluating information concerning needs; estimating and coordinating program/unit activities and associated costs; and tactical project execution activities.ESSENTIAL RESPONSIBILITIESLead the program and business-planning process across cross-functional teams within the community, creating buy-in, identifying success metrics, and assuring the program meets the metricsAnalyze client needs, changing health plan and DHCS requirements, County public health and WPC needs, current industry trends, and vendor/partner capabilitiesTranslate client needs into detailed requirements for development by the SCHIO or vendor technical teams if the need is /platform-relatedCollaborate with clients to develop effective workflows that maximize organizational productivity and that enable the organization to scale up to meet CalAIM demandsIdentify displaceable costs, where the TWC program platforms can reduce work in today's methodologyCreate TWC documents that describe business cases, high-level use cases, technical requirements, revenue, and ROIrollout of the TWC program throughout the community, coordinating activities, performing demonstrations, identifying displaced costs and benefits to clients, and maximizing usage of the programAdjust rollout plans to ensure that they are consistent with TWC , that the message is effectively conveyed, and that clients adopt and utilize the program platformsAct as a evangelist to build awareness and understandingTravel Expecation: This position will work and travel 75 - 80% within the Santa Cruz County area. It is designated as a remote function, and expected to work from home 20 - 25%. You must have the ability to work effectively from your home, including an that is conducive to productive work and relatively free from distractions, and reliable phone and internet service capable of video conferencing.QualificationsMinimum Qualifications5 years of experience in healthcare and/or social program deliveryExperience in managing multiple program issues simultaneously and leading/conducting program meetingsExperience in understanding of key program performance metricsPrevious employment longevity in full time or contract roleBachelor's Degree - Bachelor's Degree required in business administration and/or related fieldPreferred Qualifications10 years of experience in healthcare and/or social program deliveryMaster's Degree preferred in related fieldTravel Expecation: This position will work and travel 75 - 80% within the Santa Cruz County area. It is designated as a remote function, and expected to work from home 20 - 25%.
