AbilityPath is hiring a Part Time and Full Time Independent Living Skills Case Manager to join the team! The Case Manager promotes empowerment and independent living for individuals who may have physical and/or intellectual disabilities through home and community based training and supports. Promote each individual's overall goal to achieve complete independence. --- Ensure the health and safety of people receiving services, personnel and visitors at all times
--- Assess and assist in development of participant's abilities as detailed in the Individual Service Plan, as assigned
--- Must have reliable be able to travel to various sites
--- Analyze situations or tasks and break down into teachable components according to individual training needs
--- Build professional, value added relationships with participants and stakeholders
--- Act as a representative of the agency in furthering its objectives and advocating for improvements in the quality of life for persons with intellectual disabilities
--- Timely completion of all required paperwork
--- Use appropriate techniques and materials to implement participant's Individual Service Plans
--- Maintain monthly communication with individuals concerned with participant including family, service coordinator and others
--- Identify opportunities for increased independence and proactively address potential barriers to independence
--- Be prepared to employ First Aid and/or CPR in emergency situations as needed
--- Comply with participant's rights, AbilityPath and program policies, CARF standards, NPS compliance, CCL regulations, CAL-OSHA, quality assurance standards, etc.
--- Must act as a Mandated Reporter, and comply with applicable federal, state, and local regulations and laws
--- Ability to provide engaging instruction to participants with various developmental levels, skill sets, and behaviors
--- Ability to respond to challenging behaviors and support behavioral plans to increase engagement
--- Awareness of, sensitivity to, and competence in supporting people with developmental disabilities Other duties as assigned EDUCATION AND WORK EXPERIENCE:
Any combination of education and experience that would likely provide the required knowledge, skills and ability is qualifying, unless otherwise specified.
--- Associate's or Bachelor's Degree in relevant field
--- Experience working with adults with developmental disabilities HIRING REQUIREMENTS:
Valid California Driver's License/ID
Clean Driving Record (if applicable)
Proof of negative TB test results
Verification of physical clearance
Passing a criminal fingerprint clearance
18 years of age or job permit clearance
Proof of Covid-19 Vaccination and Booster PHYSICAL ACTIVITY: Accommodations made upon request to Human Resources
--- Ability to physically respond to adults with developmental disabilities
--- Ability to lift/support at least 40 lbs as needed
--- Ability to sit, stand, stoop, and bend frequently
--- Ability to walk for extended periods of time and support adults with developmental disabilities on community outings
--- Ability to drive company vehicles to transport participants
