LanceSoft is seeking a travel Physical Therapist for a travel job in Hayward, California. Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: ASAP - Duration: 13 weeks - 40 hours per week - Shift: 8 hours - Employment Type: Travel PT: Field position-Hayward/ Fremont Home Health Tues-Sat or Wed-Sunday LOCALS OK Home Health experience preferred- willing to consider candidates with no HH experience CA license in hand preferred Oasis experience required GWW hours are based on completed visits: 25 visits = 40 hours Hours are guaranteed if 25 visits or more are completed COVID Exemptions permitted Mileage between patients- IRS Standard Please Note: Rate may change based on coverage territory and experience Accepting pending CA license. Traveler need to have applied upon submission 1. Experience Requirements: - Minimum of 1 year of Home Health experience - OASIS experience PREFERRED - Locals at travel rate - BLS- AHA required - Timely communication and documentation are mandatory 2. CA License in-hand required with submission 3. All travelers are required to have an active drivers license, and personal transportation 4. RTO (Requested Time Off) must be submitted at the time of application 5. On-call rotation may be required- please discuss during interview 6. Home Health Checklist is mandatory 7. Travel Pairs: accepted, however each traveler will be assigned to their own territory and must have own transportation 8.
