Responsibilities

The Stroke Program and Clinical Documentation Program Coordinator is responsible for the management of two clinical programs: the Accredited Primary Stroke Center and Clinical Documentation Improvement. With both programs, the Coordinator is the leads the hospital departments, physicians, external agencies, nursing staff, and allied healthcare professionals. The Coordinator ensures the care for the stroke patient throughout the continuum and works with the Case Management department and other health team members to provide comprehensive medical record documentation to reflect clinical treatment, decisions, and diagnoses for inpatients. The Coordinator is responsible for assuring Sequoia Hospital's continued certification as a primary stroke center and for supporting Sequoia in assuring accurate and complete clinical documentation. The Coordinator will include education, reporting and marketing as appropriate for the stroke and documentation programs.

The coordinator ensures quality and regulatory compliance for the Stroke and the Clinical Documentation Improvement Program providing education and development opportunities for staff as appropriate. As the Clinical Documentation Improvement Program Coordinator, the individual will use coding and clinical expertise to identify opportunities for staff and physicians to and ensure accuracy and completeness of clinical documentation used for measuring and reporting physician and hospital outcomes. This position serves as a liaison for Information Management Coders and Patient Financial Services in the compliant coding and processing of inpatient claims. The Coordinator works under the direction of the Vice President of Patient Care/Chief Nurse Executive. All employees are expected to perform their duties in alignment with the vision and values of the organization.

The person doing this job is responsible for a variety of tasks that lead to a high level of customer satisfaction in the most costeffective manner with respect and dignity Incumbents in this position shall comply with all applicable laws and rules including the Dignity Health Standards of Conduct and Dignity Health policies and procedures that apply to his/her job responsibilities, seek guidance when in doubt, promptly report any potential or suspected violation of Dignity Health Standards of Conduct, Dignity Health policy or procedure or applicable laws/regulations and when requested, assist Dignity Health personnel in investigating all allegations of violation.

When called upon, the incumbent shall assist in risk evaluation and work collaboratively with facility/entity leaders to implement, monitor and mitigate risk corrective plans outlined by the Compliance program.


Qualifications


Required:

  • Experience with coding procedures and/or DRGs is required.
  • Clinical experience in care for stroke patients is highly desirable.
  • Three to five years experience in teaching and/or clinical program development and coordination is desirable.


Education

  • BA/BS or equivalent required.


Licensure:

  • Current California Registered Nurse
    License required (RN)
  • Current Basic Life Support certification for healthcare providers required. American Heart Association
  • CT CPRBLS


Preferred:

  • Masters Degree preferred.
  • Experience in data collection and statistical analysis preferred.
