Job Description:
AbilityPath is hiring a Direct Service Instructor to join the
Tailored Day Services team! The TDS Instructor works one on one
with participants who are interested in pursuing employment,
education and community access goals within the community. Working
closely with individuals, you will assist them to design their own
customized program based on their goals, needs and interests.ABOUT

participants to develop a person centered plan and develop a
customized program to reach his/her goals, assesses participant's
strengths, interests and challengesSupport participants to find and
participant in volunteer activities, community and work-related
activities, and community college classes to develop skillsProvide
customized programming to individual participants. Programming may
include community based programming such as: volunteer
opportunities, college classes, tutoring, travel training etc.;
vocational activities including: job discovery, informational
interviews, employer research, job searches, job shadowing, job
coaching etc.Provide engaging, developmentally appropriate
instruction for participants as neededContribute to networking with
employers in the community to set up and arrange informational
interviews, employer tours, as well as identifying job leads and
employer contactsUtilize appropriate modification techniques to
respond to challenging behaviors and support behavioral plans to
increase engagementResponsible for participant case management:
schedule and facilitate participant annual meetings, complete
documentation for tracking participant goals and comment on
progress and achievementMaintain professional relationships and
provide excellent customer service with participants, employers,
external parties, families, and care giversDrive agency vehicles as
needed (transporting participants to events, etc.)Responsible for
daily exterior inspections of agency vehicles when
applicableResponsible for making sound decisions in real time on
community-based activities and safety/well-being of participants
while out in communityOther duties as assignedEDUCATION AND WORK
